For insurance and legal reasons, this form must be completed prior to the player(s) participating in a training session or match on or after 1st July 2025.
A payment must be made online as the final step of your application. You will not be able to complete the application without a valid credit or debit card.
Prior to payment, you will be given an opportunity to review and print a copy of the details you have entered. If you wish to keep a copy for your own records, you should do this by clicking the Print button.
You will be emailed a reference number and a payment receipt upon completing the application.

Cancellation Policy
If, for any reason, you wish to withdraw your membership of the club within 4 weeks of joining, then the fee will be refunded less the cost of any kit received, unless returned unused.
